• Purchase Order Number
• Date ordered and date required. (Please indicate urgency)
• Product Manufacturer, item or model number, quantity and price.
• Please indicate the "Ship to" and "Bill to" address's (if different)
• Contact information including name and phone number
• Please include shipping company's name and your account number. If you wish to have items shipped prepaid and collect we will ship via Canpar from or Aurora location.
• Indicate shipping service required such as Air, Overnight, 9 am etc.
